To make an appointment to review your medical records in our 医疗记录 office, please call (415) 353-2221 during our office hours.
We charge 25¢ per page to copy medical records or $25 if using an electronic record. We will call to notify you of the total charge. You may mail us a check or pay the fee in person if you pick up your records.
Radiology Imaging and Reports
Your imaging test will be interpreted by a UCSF radiologist, a doctor trained to interpret imaging exams. The radiologist will write a report with the results of your exam. After the report is finalized – typically within two to five working days – there are two different ways you can access this report and receive your results:
The report and the images from your exam will be available in MyChart. This is the fastest way to receive your results. Please note: Because your results are 可以在MyChart immediately after they are finalized by the radiologist (effective March 31, 2021), your healthcare provider who ordered the study may not have reviewed these radiology results yet.
The report and images are also available by CD, free of charge. To request a CD, complete the Authorization for Release of Health Information form. (Form for Spanish-speaking patients: Autorización de divulgación de información médica.)

A health information exchange (HIE) allows your health care providers to use secure, electronic methods to share your most recent medical information with participating doctors' offices, 医院, 实验室, radiology centers and other providers participating in your care. The purpose is to give each of your participating caregivers access to the most up-to-date information available.
病人 are automatically set to opt in to HIE, meaning information will be shared with participating physicians and health care providers unless you provide instructions to opt out.
